Home Care Services

Care_Options_Image6UPA Home Care Western Region (Dubbo and Yeoval) offer the following services;

Home Care Packages (HCP)

Consumers assigned a Home Care Package will be allocated a Case Manager who will work with you to develop a care plan to meet your assessed needs and goals. Within government guidelines, you can choose to spend your package funds on support, equipment, and services that relate to your health, well-being and care.

This can include; assistance with showering and dressing, domestic assistance such as cleaning essential areas of the house, laundry and bed making, assistance with shopping, escorted transport to appointments, meal preparations and assistance with special dietary needs, basic garden and lawn maintenance, medication assistance, in home respite, and access to social activities and programs.

Your Case Manager can also help you to ‘think outside the box’ and consider some options you may not have considered, but could assist you to remain independent. This could include; spring cleaning, professional laundering, podiatry, counselling, speech pathology, taxi vouchers to enable independent social access, mobility training to safely use public transport, basic cooking classes, payment towards some home modifications, personal support whilst travelling, exercise programs and classes like yoga, hydrotherapy and massage.

Home Care Packages are all about tailoring care to suit the needs of the consumer. We know that no two people are the same and we look forward to tailoring a care plan to suit your individual needs

Commonwealth Home Support Programme (CHSP)

The Commonwealth Home Support Programme launched in July 2015, combined several other Government funded initiatives such as the National Respite for Carers Programme, Day Therapy Centres Programme, Commonwealth Home and Community Care, and the Assistance with Care and Housing for the Aged Programme to provide an entry level suite of services.

The focus of the Consumer Home Support Programme is working with you, rather than doing for you, to help you maintain your independence and lifestyle. The accessibility to CHSP depends upon your needs and, the programmes funded in each region. This varies from the HCP which is a multidimensional, personally tailored plan of care and services.

UPA Western Region (Dubbo and Yeoval) is funded to provide the following CHSP programs;

Domestic Assistance- Helping you around the home with tasks such as laundry, bed making, dusting, vacuuming and cleaning essential areas.

Social Support Groups– Safe environments operating Monday to Friday to provide a break for carers and to provide a social outlet for consumers. This group service includes meals and snacks, tailored activities and outings to suit the individual.

Veterans Home Care (VHC)

Veterans Home Care is a program funded by Department of Veterans Affairs for eligible servicemen and women. Like other home assistance packages, Veterans Home Care is designed to help consumers stay live independently within their own community.

The Department of Veterans Affairs assesses consumer for their care needs, and an approved service provider provides the approved services. We are happy to work in conjunction with Department of Veterans Affairs to assist our servicemen and women.

Private Services

UPA’s private services allow people who are either unable, or do not wish to access funded services to receive services to help maintain independence and wellbeing.

The available services include personal care, domestic assistance, transport to and from appointments and other services tailored to suit your needs.

Brokerage Services

Not all service providers are able to provide reliable staff in all areas. We are incredibly fortunate to have dedicated staff in the cities and small towns we serve. Sometimes our staff are brokered out by other services when they are unable to use their own staff with UPA Dubbo being the preferred agency in many instances. This is one of the ways we work within the community to help ensure you get the best care you can, when it suits you.

You’ll find our office on the main road in Dubbo, overlooking the Macquarie River. At our beautiful and historic Lillimur House, we offer the Lillimur Social Club -a centre-based Day Care on Monday, Wednesday and Friday, that has become an integral part of the community, offering social activities to our clients and free time for Carers.
