About Us

Our office is situated in the UPA Sydney West District, Melrose Village in Pendle Hill.

Sydney West Community Care commenced in November 2007 with 30 Community Aged Care Packages (CACP) funded by the Australian Government, Department of Health and Ageing. Growth of our community program has continued in the past twelve months with an additional 22 community aged care packages being awarded in August 2009.

Our Community Aged Care service received a very positive outcome from the Department Quality Review Team at the Quality Reporting site visit in April 2009.

What are Community Aged Care Packages (CACP)?
UPA CACP clients will receive flexible services designed to meet their individual assessed care needs.

Services may include:

  • Case Management
  • Personal Care
  • Light housework
  • Shopping assistance
  • Meal preparation and assistance
  • Transport
  • Light gardening
  • Socialisation
How can I access a Community Aged Care Package?
To access CACP, you must first be assessed by an Aged Care Assessment Team (ACAT).

The ACAT is usually based at a hospital or community centre and may include a doctor, nurse, social worker, physiotherapist or occupational therapist.

Our Staff
Our staff are committed and dedicated to providing excellent care for our clients and we endeavour to ensure that we attract, recruit and retain the best possible people. Our staff turnover rate of 4% for the past year is an excellent result and well below the industry average.

Our learning and development program offers career development opportunities with care staff encouraged to undertake Certificate 4 training. All staff are supported and developed with a wide range of training opportunities delivered throughout the year, including access to the Aged Care Channel programs.
