Community Care Program

Community Care Packages are a planned and co-ordinated program to provide an alternative service for frail aged people whose dependency and needs would qualify them for residential care.

Our community based care programme has a total of 17 places for those who prefer to stay in their own homes and receive assistance with their day to day life.

Our skilled carers can help with most of the chores around the home and generally offer their assistance to allow people to maintain their independence for as long as possible.

ACAT assessment is required before participation in this scheme.

  • Personal Care - Bathing/Showering, oral hygiene hair & nail care, toileting, dressing/undressing.
  • Mobility - Transfers, assistance with walking aids.
  • Meals - Meals on wheels, special dietary needs catered for.
  • General Housework - All house cleaning, bed making, linen change, laundry, ironing.
  • Gardening - Gardening and minor house mtce
  • Supervision of medications
  • Minor Wound Dressing
  • Transport - Medical appointments, visiting friends, shopping, general outings (via care plan).
  • Socialisation - Group activities or individual outings.
  • Support Services - to maintain personal affairs eg. Banking, business appointments.
