Community Care program
UPA Murray River Community Aged Care Packages (CACP) aims to provide
people with the opportunity to remain in their own home and continue
to be part of the community in the way they have always been. This
enables them to remain contributors to the important network already
developed, recognizing both their dignity and their value for our
society. UPA Murray River CACP’s has 20 packages in
Lavington and 5 Henty.
To be eligible for the programme people need to have been assessed
by the Aged Care Assessment Team (ACAT). Once the ACAT assessment
has been completed and deemed eligible they are considered for
admission to the programme.
The programme focuses on each person’s individual needs
and is designed specifically for the person it is servicing. Services
that can be offered are personal care, light housework, laundry,
shopping, day care, appointments, mobility, meal preparation and
medication monitoring.
