UPA PRIVACY POLICY STATEMENT
- We recognize that we hold a position of trust in holding your
personal and sensitive information.
Information will only be collected by UPA as and when it is required
to provide a service or meet regulatory requirements.
- We respect
the right to privacy of each resident/client/staff/volunteer.
- We
will maintain all personal information in a safe and secure place
with confidentiality respected at all times.
- Once the statutory
time limits for storage of information has passed, records will
be destroyed in a secure manner.
- We will make sure that access
to information held by UPA is available to each individual. Please
contact the local UPA office to arrange access.
- We will make
sure that all staff treat our residents/clients and other staff
information in the strictest confidence.
- We will make sure that
all personal and sensitive information and data collected will
only be released to the following people:
- Resident/client/staff/volunteer/themselves.
- Other health
professionals – if the need arises and
as required.
- Family members or other persons designated as
the "person(s) responsible" for
the affairs of the resident/client/staff members.
- Statutory
bodies as required - the person whose information is to be
passed on will be informed.
- Any concerns or complaints will
be treated confidentially and impartially and will be dealt
with promptly.
